Applying for your first (initial) teacher certificate:

Candidates who are applying for their first (initial) teaching certificate will complete and submit the UK College of Education certification form, the Kentucky EPSB TC-1 application, and a UK transcript request form. Printed copies are available in 166 TEB, or can be printed from the web sources below.

Once completed, the forms should be returned to the Office of Academic Services & Teacher Certification (166 TEB). Note: Incomplete application materials will not be processed. Questions should be directed to Martha Geoghegan (859) 257-7681, myrt@uky.edu.

Applying for a rank change, an endorsement, educational leadership, extensions, school media librarian, or school psychology:

Candidates who are applying for advanced certification or for additional certificates will fill out the EPSB's TC-1 form, the accompanying UK College of Education advanced certification form, and a UK transcript request form. Paper versions are available in 166 TEB, or can be printed from the web sources below:

Applying for a one-year, temporary certificate as part of the Alternative MSD certification program:

Candidates who are applying for a one-year temporary certificate enabling them to teach in a Moderate to Severe Disorders classroom for one year as part of the three-year Alternative Certification MSD program at UK will complete and submit the UK College of Education certification form and the Kentucky EPSB TC-1 application. Printed copies are available in 166 TEB, or can be printed from the web sources below.
If this is your first year in the Alternative MSD certification program, and you have not attended any UK classes yet, you do not need to complete a UK transcript request form. If this is any other year of the Alternative MSD program or if you have UK course work, you must also complete the UK transcript request form.
